Feedback form and results

Building the form: Admin → Feedback Form. Add questions of four types: rating, multiple choice, short text, or long text (textarea). Each question can be marked required, and separately marked anonymous (so you can see the answer without seeing who gave it). Entrants reach the form via "Leave Feedback" on their own menu.

Viewing results: Admin → Feedback Results. Rating and choice questions are summarised as counts per option; text and textarea questions are listed as individual answers. There's no per-respondent breakdown shown for anonymous questions, just the aggregated answers, matching what you marked anonymous when building the form.
